Then, the relevance between these factors and services was clarified.\nThrough the germination stage in which the services was shouldered mainly in respect of medical consultation and hospitalization support of the patient who is distressed by poverty and injury and sickness with the viewpoint of the local community including the family, the viewpoint to the local community was diluted by public health care center medical social work retreat after the GHQ withdrawal after the world war 2. At the same time, the emphasis on social work methodology and theory increased. Though established occupational organization required quality improvement and services clarification, it deviated from the initial purpose and devoted to the qualification movement.\nThe policy promoted basic structural reform, earlier discharge from hospital and transition to the regional community in order to deal with the financial resource expenditure control by the low birth rate and aging society.\nThe medical social work services became inward by backing to policy and organization, and the viewpoint which made the life problem which individually appeared to be a problem of the society was
