This is because there is indication that the difference of the remuneration between a medical specialist and a general practitioner has brought about a medical specialist's shortage. This comparison was performed in five nations, the United States, Britain, France, Germany, and Japan, based on the data of OECD, etc. This investigation showed that a Japanese general practitioner's remuneration was in a very high level. On the other hand, it turned out that a Japanese medical specialist's remuneration is a low level. From this, it was suggested that the difference of the remuneration between a Japanese medical specialist and general practitioner is a reason for changing a career to a general practitioner from a medical specialist. However, Decreasing required expense, when a general practitioner opens an individual clinic is a policy to which priority is given rather than lowering a general practitioner's remuneration. On the other hand, it is necessary to raise a Japanese medical specialist's remuneration
