 note = {本研究ノートは、日本の医師不足の理由のひとつである病院勤務医の診療所開業医への転出が、両者の報酬格差にあるとの指摘に対して、わが国と欧米先進国の医師のキャリアとその報酬水準を比較し、政策について検討したものである。報酬水準の比較は、先行研究にならって、主にOECDによるデータを基に、アメリカ、イギリス、フランス、ドイツの4カ国を対象に、職務形態である専門医と総合医の別、雇用形態である自営医と勤務医の別に日本の勤務医と開業医との間で行った。その結果、日本の開業医の報酬は、著しく高い水準にあること、それに対して日本の勤務医の報酬は低い水準であることが確認され、報酬格差は転出の理由となっていることが示唆された。しかし、開業医の高い報酬水準は、多額の個人負債で開業せざるを得ない事情を考えれば可処分所得でみると著しく高いとは断言できず、直ちに開業医の報酬水準を下げるのではなく、稼働率の低い医療機器などの開業コストを見直す政策を優先すべきであるとの結論を得た。他方、勤務医の報酬水準は高める必要があるとし、特に過重労働とされる産科、小児科、外科、麻酔科などに従事する病院勤務専門医への報酬加算が、適切な病院内の人事考課制度によって配分される必要があるとの結論を得た。, This research compared the doctor's career and remuneration level of Japan and the West. This is because there is indication that the difference of the remuneration between a medical specialist and a general practitioner has brought about a medical specialist's shortage. This comparison was performed in five nations, the United States, Britain, France, Germany, and Japan, based on the data of OECD, etc. This investigation showed that a Japanese general practitioner's remuneration was in a very high level. On the other hand, it turned out that a Japanese medical specialist's remuneration is a low level. From this, it was suggested that the difference of the remuneration between a Japanese medical specialist and general practitioner is a reason for changing a career to a general practitioner from a medical specialist. However, Decreasing required expense, when a general practitioner opens an individual clinic is a policy to which priority is given rather than lowering a general practitioner's remuneration.
