Through analyzing the cases of fragmented care, the study considered the challenges in the existing supply system and presented requirements for establishing a care management system that addresses those challenges. As a result, it was shown that a care management system is necessary in the community-based elderly care service supply system, with three functions of assessment, service coordination, and monitoring needing to be established.
